Brook-Falls Luxury Resort & Doggy Day Care Enrichment Doggy Day Care/ Training & Lodging Policies Waiver of Liability, Release and Indemnity Agreement
Owner Representations

By enrolling my pet to receive these Services, I represent that I am the legal owner of my pet. I further represent that my pet is in good health, that it has received its necessary vaccinations and preventative care, and that I am unaware of any violent or aggressive tendencies my pet may have exhibited in the past to people, or animals. I agree to make Brook-Falls Luxury Pet Resort & Doggy Day Care aware of any injuries, allergies, medical ailments, physical limitations, and pre-existing conditions before the start of any Services. I understand that Brook-Falls Luxury Pet Resort & Doggy Day Care reserves the right to dismiss or refuse admittance to any pet that does not meet health, temperament, or other standards for these Services. This determination shall be made at the sole discretion of Brook-Falls Luxury Pet Resort & Doggy Care.

All dogs must pass a Temperament Evaluation
All dogs must pass a Temperament Evaluation prior to acceptance into the Enrichment program. The Brook-Falls Luxury Pet Resort & Doggy Day Care employees will not knowingly admit any obviously aggressive dogs into the program.
All vaccinations must be current
All vaccinations must be current in order to participate in lodging, enrichment and training classes. I am also aware that while very effective, vaccines (in particular but not limited to the Bordetella vaccine) do not protect against all strains of infectious organisms. For this reason, I will not hold Brook-Falls Luxury Pet Resort & Doggy Day Care responsible for any occurrences of socially transmitted or airborne illness.
All participants must be current on flea/tick prevention
To reduce the risk of external parasite transmission, all participants must be current on flea/tick prevention. If any pet is found to have external parasites, Brook-Falls Luxury Pet Resort & Doggy Day Care will bathe the pet and give a single dose of prevention at the client’s expense.
All dogs over the age of 7 months must be spayed or neutered
I understand that all dogs over the age of 7 months must be spayed or neutered to participate in group play activities.
Keep our facility clean and sanitary
Brook-Falls Luxury Pet Resort & Doggy Day Care strive to keep our facility clean and sanitary and uses all reasonable precautions to prevent injury, illness and/or escape. Under no circumstances will Brook-Falls Luxury Pet Resort & Doggy Day Care be held responsible for the action of the pet or other pets in the play group that may cause injury, illness or escape.
Well-being and happiness of every dog
Brook-Falls Luxury Pet Resort & Doggy Day Care will practice diligence to ensure the safety, well-being and happiness of every dog in our playgroups. Owners understand there will be certain risks associated with allowing dogs to interact freely with one another. Even with constant supervision, nips, scratches, muscle soreness, dog fights or contagious disease may occur.
Not responsible for any personal items
Brook-Falls Luxury Pet Resort & Doggy Day Care is not responsible for any personal items such as bedding, toys or leashes left while your pet is in our care.
Wet when you pick them up
Your pet’s coat may be wet when you pick them up during the summer. On nice days, they may have been lounging in the pools. Not to worry, the water is changed daily to keep them clean and healthy.
Considered abandoned
I understand that if my pet is left with Brook-Falls Luxury Pet Resort & Doggy Day Care for longer than 10 days after my scheduled pick-up with no contact from me, my pet will be consider abandoned. After the 10 day hold period my pet will be transferred to HAWS animal shelter for adoption. If Brook-Falls Luxury Pet Resort & Doggy Day Care is able to contact me within that 10-day time period, I acknowledge that I will become finically responsible for any additional lodging fees associated with my pet’s extended stay.
Photo Release Waiver

I grant to Brook-Falls Veterinary Hospital & Exotic Care, Inc & Brook-Falls Luxury Pet Resort & Doggy Day Care, its representatives and employees the right to take photographs of me and/or my pet, and to copyright, use and publish the same in print and/or electronically.

I agree that Brook-Falls Veterinary Hospital & Exotic Care, Inc. & Brook-Falls Luxury Pet Resort & Doggy Day Care may use such photographs of me and/or my pet with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising and web content.
Medical Authorization
If my pet becomes ill or injured while participating in any services provided, Brook-Falls Luxury Pet Resort & Doggy Day Care will attempt to contact the emergency contact provided to us. However, such an emergency might not provide the time to do so prior to the administration of care. I authorize Brook-Falls Luxury Pet Resort & Doggy Day Care to provide care for my pet and to transport my pet to a 24-hr referral facility if our veterinarians deem such medical care to be important to my pet’s health. I grant Brook-Falls Luxury Pet Resort & Doggy Day Care and its employees’ full authority to make decisions involving the medical treatment of my pet and agree to pay for all related costs. This applies to any claims for injuries or damages related to such medical care or that arise due to the transport of my pet.
In the event the owner or emergency contact cannot be reached and the pet requires emergency treatment, Brook-Falls Luxury Pet Resort & Doggy Day Care will seek treatment for the pet and the owner will assume full financial responsibility under the guidelines authorized below. - Please choose only one option.

  • IN ADDITION, BY SIGNING BELOW, I RELEASE BROOK-FALLS LUXURY PET RESORT & DOGGY DAY CARE FROM ANY AND ALL LIABILITY FOR ANY INJURY OR DAMAGE THAT MAY ARISE FROM, BUT NOT LIMITED TO, THE ACTIONS OF ANOTHER PET WHILE MY PET IS ON THE PROPERTY. I, RELEASE AND WAIVE ANY AND ALL CLAIMS AGAINST BROOK-FALLS LUXURY PET RESORT & DOGGY DAY CARE, AND FURTHER HOLD HARMLESS AND INDEMNIFY SUCH RELEASEES FROM AND AGAINST ANY LIABILITY, CLAIMS OF NEGLIGENCE, MISADVENTURE, HARM, LOSS, INCONVENIENCE OR DAMAGE HEREBY SUFFERED OR SUSTAINED AS A RESULT OF MY PET’S PARTICIPATION IN THE SERVICES OFFERED. THIS IS NOT A RELEASE AND WAIVER OF INTENTIONAL OR RECKLESS ACTS OF BROOK-FALLS. SUCH RELEASE, WAIVER, HOLD HARMLESS AND INDEMNITY SHALL APPLY TO MY OWN CLAIMS AND/OR CLAIMS OF THIRD PARTIES, RELATING TO MY PET’S PARTICIPATION IN THE SERVICES.

    I HAVE READ AND UNDERSTAND THIS AGREEMENT. I HAVE HAD THE OPPORTUNITY TO BARGAIN WITH BROOK-FALLS LUXURY PET RESORT & DOGGY DAY CARE TO NOT HAVE TO AGREE TO THE WAIVERS OR RELEASES OF LIABILITY OR INDEMNIFICATION PROVISIONS SET FORTH I HAVE SIGNED THIS WAIVER VOLUNTARILY, IN EXCHANGE FOR THE CONSIDERATION DESCRIBED ABOVE.

    This agreement and its terms bond a relationship between Brook-Falls Luxury Pet Resort & Doggy Day Care and yourself. Each time your pet (s) enters the Brook-Falls Luxury Resort & Doggy Day Care you affirm the terms of this agreement and agree to indemnify and hold Brook-Falls Luxury Pet Resort & Doggy Day Care and its employees harmless from all liability and claims.

    The Services are provided under this Agreement by Brook-Falls Veterinary Hospital & Exotic Care, Inc.
